How to apply for supported living?

Living in your own house is a significant step towards personal growth, development, and independence. While it can be a daunting decision, it is a milestone that many people strive for. With the right mindset and transition preparation, it can be a fulfilling and rewarding experience. The information below details how you can apply for supported living.

Contact Adult services

Your first step is to speak to your Social Worker. If you don't have a social worker you can ask for one to be allocated through your GP or you can self refer. In Northumberland this is done via One Call:

choose a care provider

The Local Authority and Social Work teams have a list called the Provider Framework - this is a list of approved care providers that are operating in your area. You should take your time exploring them and choosing the right one to meet your needs. Once you have identified a provider, speak to your Social Worker and they will make the arrangements for setting up your new service.

Needs assessment

Once you have made a referral to Adult Services you will be allocated a key worker, this will usually be a Care Manager or a Social Worker. They will come out to meet you (and your family/carers) and they will then complete what is called an Adult Needs Assessment - this is how they assess how much support you will need in your new home. They will then submit a request for your own personal budget.

If you choose natural ability

If you choose Natural Ability, and it is approved by Adult Services, a referral can be made. This can be made by yourself or your Social Worker.

In order to ensure Natural Ability is the correct service for you we encourage you to contact us to have an informal discussion or arrange a visit.

This will give you an opportunity to find out more about the ISL Service and discuss your needs. If you would prefer a family member can do this on your behalf.

The Senior ISL Manager will be able to provide you with all of the information you need.

Access housing

Finding a house can be daunting but there are people who can help you. Your local council will have a housing application process you can apply to; your Social Worker may refer you to be on the 'complex housing panel' which is where professionals will try and support you to find an appropriate property; or alternatively you can look at private housing. Once a property is identified, you and your Social Worker will look at care providers.

Transition to ISL

Once your service has been confirmed the ISL management team will work with you, your family and the professionals to agree a transition plan.

Although different for each individual, this plan may include recruitment and meeting of your core team, transition days with your team (this may be in our Towards Independence Training Suite), timescales for your house move and support with your final transition into your new home, which could be phased to best meet your needs.
